New Student Registration Form

Any students who may be taking lessons off MISU campus must submit this waiver form. This form is for use AFTER you have submitted an inquiry form and been contacted by a MISU staff member. Please complete a separate form for each student enrolling in lessons.

This form is also used for new students to register for a group class, see bottom of form. To register for Community Orchestra, please fill out the orchestra registration form linked here.

  • Receipt of full payment is required PRIOR TO 1st lesson. Please note that our PayPal and Venmo options are not available for the summer session.
  • By submitting the form above I understand and agree to the following: 1. Tuition is non-refundable; 2. All students participating in the lessons and any other activities offered by MISU are participating at their own will and assume all the potential risks, dangers, hazards, and responsibilities surrounding such participation. MISU is not, and will not be, responsible in any way for any risks, hazards and dangers (including loss of property, personal injury or death) incurred to any student/parent/guardian/or other during participation;3. MISU and its teachers invest substantial effort to provide high-quality lessons and to make sure that each student receives the attention he/she deserves. The student understands and agrees that he/she will make all reasonable efforts to arrive to the lessons on time and in the event that he/she cannot make it, to provide notice of his/her tardiness or absence ahead of time. Any missed lessons will only be made up or re-scheduled in accordance with the provisions below; 3.1. Lessons missed by students will neither be refunded, made up, nor applied to any subsequent periods. Notwithstanding the above, in the event that the student provided sufficient notice in advance, the teacher, in his/her full discretion can suggest a potential make-up lesson. “Sufficientnotice in advance” shall mean at least 48 hours before the lesson; 3.2. In the event that a teacher is absent the lesson will be re-scheduled; 3.3. In the event of two or more cancellations due to inclement weather, the teachers will apply reasonable effort to re-schedule the missed lessons; 3.4. The time and place of any re-scheduled lessons will be made at the teacher's full discretion; 3.5. The above cancellation policy applies to all teachers and missed lessons unless a different arrangement has been explicitly agreed upon in written form between the student and the teacher. Such arrangement will only apply for such specific lesson with that specific teacher.
